How to Apply

To file a claim, provide the Motor Vehicle Dealer Board with the following:

1. A complete and notarized Judgment Claim Request form
2. A copy of the extended motor vehicle warranty if the claim is on an extended warranty (applies only when a dealership has been originally licensed on or after April 8, 1994, and the breach occurs during the first three years of the licensed period)
3. A copy of the check made payable to the dealer or a receipt attesting to payment, and a copy of the bill of sale
4. A statement as to the location of the vehicle's certificate of title
5. An attested copy of a final court judgment
6. An affidavit telling all the facts of the claim and explaining what the dealer or salesperson did that resulted in fraud or violated the Virginia Motor Vehicle Dealer Laws
7. Your social security number or employer identification number
